BCS OFF CAMPUS ACTIVITIES

PERMISSION SLIP 2008-2009

My child has permission to attend all school sponsored activities for the school year. These activities will include, but are not limited to, field trips and local activities such as public library use, play at B-W schools, and athletic activities such as sledding and skating. Parents will be notified prior to each event other than regularly scheduled trips to the library. I understand that BCS drivers are all volunteers and they assume no liability.
